Scientific Collaboration Network Survey

This is a collaboration survey used with research investigators involved in team science initiatives. It includes a network component and works best with a known network, as it asks respondent to indicate whether they currently work with another investigator or whether they have worked with them in the past. It was initially used for an evaluation with the Centers for Population Health and Health Disparities (CPHHD) funded through the National Cancer Institute and the National Health, Lung, and Blood Institute (NHLBI), but has been modified and adapted since for other grant-funded team-based research initiatives.
